Please see abov letter from the HSE which I believe confirms their neglect. This HSE letter confirms they investigated a suspicious death of a Marks and Spencer employee. However, upon completion of their inquiry they hand over their report to a 3rd party and fail to retain copies for their own files .I don't believe this is right and proper for a public authority and there is no doubt the HSE is a public authority.

Such actions are not acceptable in my view because this death was related to H&S issues. Hence, the HSE have a duty of care to retain public records.
I call upon my MP, Ben Bradshaw to look into this matter and ask the HSE why they do not retain public records for such matters.
